Derivation and validation of a novel preoperative risk prediction model for surgical site infection in pancreaticoduodenectomy and comparison of preoperative antibiotics with different risk stratifications in retrospective cohort

背景

手術部位感染（SSI）はよく見られる膵頭十二指腸切除術の術後合併症である。

目的

転帰を改善し、術前抗菌薬の指針となる可能性のある SSI リスクの術前特定モデルを開発すること。

方法

メタアナリシスによって予測モデルを構築した。文献検索および採択、データ抽出、定量的統合を行った後、予測因子の統合オッズ比に基づいて予測モデルを確立した。単一施設後向きコホートを検証コホートとした。受診者動作特性曲線と曲線下面積を用いてモデル性能を評価した。決定曲線と較正プロットも作成し、ノモグラムを評価した。異なるリスク層別化による多変量ロジスティック回帰を用いて、予防的抗菌薬の SSI に対する効果を群間で比較した。

結果

28 件の研究をメタアナリシスの対象とし、17 件を誘導コホートの対象とした。年齢、男性、体格指数、膵管の直径、高リスクの診断、術前の胆道ドレナージを選択し予測モデルを構築した。本モデルは外部コホートで検証した。開腹膵頭十二指腸切除術におけるカットオフ値は 3.5 であり、曲線下面積（AUC）は 0.76 であった。腹腔鏡下膵頭十二指腸切除術におけるカットオフ値は 4.5 であり、AUC は 0.69 であった。決定曲線と較正プロットによって、特に開腹膵頭十二指腸切除術における本モデルの有用性の高さが示された。多変量ロジスティック回帰は、異なるリスク層別化において、SSI に対する広域スペクトラム抗菌薬と狭域スペクトラム抗菌薬の間の差を示さなかった。

結論

本モデルによって、手術前に SSI の高リスク患者を特定することができる。異なるリスク層別化の下の予防的抗菌薬の選択については、さらに検討すべきである。
