背景
栄養剤は医療従事者によって手洗いシンクから廃棄されることが研究で示されており、この行為は、カルバペネマーゼ産生腸内細菌目細菌(CPE)などの病原菌を含む、細菌の増殖を促進する可能性がある。院内での CPE のアウトブレイクおよび感染は、患者と病院にとってマイナスの結果をもたらす。
目的
栄養補助飲料(nutritional support drinks:NSD)および経管経腸栄養剤(enteral tube-feed products:ETFP)が CPE に及ぼす可能性のある増殖促進効果を検討した。
方法
異なる CPE 株 6 株を、5 種類の希釈 NSD、5 種類の希釈 ETFP、ミュラーヒントンブイヨン(MHB)、M9 最小塩類培地で発育させ、すでに液体が存在する U ベンドへの少量の栄養剤の廃棄をシミュレートした。0 時間、6 時間、24 時間で CPE を計数し、二元配置分散分析および Dunnett 検定により、信頼水準を 95%として比較した。Spearman の r を用いて、栄養剤の成分濃度と CPE 増殖との関連の強さを評価した。
結果
すべての NSD および ETFP が CPE 増殖を促進し、M9(陰性増殖対照)と MHB(陽性増殖対照)のいずれにおける増殖も上回った。いくつかの例では、NSD/ETFP における増殖が MHB における増殖と比較して有意に大きかった。
結論
栄養剤は in-vitro 条件下で CPE の増殖を支持する。CPE は排水配管内で生き残る傾向があることから、不適切な製剤の廃棄によって、これらの環境リザーバーに定着した CPE にさらに栄養が与えられる可能性があることが示唆される。希釈 NSD および ETFP で増殖が認められたことから、これらのリザーバーからの CPE 伝播の潜在的リスクを軽減するためには、修正可能な行為を最適化すべきであることが示されている。
監訳者コメント:
病院環境の様々な設備は院内感染病原体のリザーバーや感染経路ともなりうる。配管や蛇口は細菌の温床となりやすく、特に手洗いシンクは直接感染伝播と関係する。手洗いシンクは手洗い専用であり、手洗い以外に患者由来の液体を廃棄することは禁止されているのが一般的であり、ましてや栄養剤を廃棄することは厳禁である。しかしながらそのような行為が実際に観察されていることも事実であり、結果的に他の医療環境にも拡大する結果となる。また手洗いシンクの排水管の U 字部分は臭気逆流防止のために排水を一時的に溜めるようになっており、CPE は U 字部分でバイオフィルムを形成し、格好の CPE の定着増殖場所となり、容易に除去は不可能となる。
