背景
バンコマイシン耐性エンテロコッカス・フェシウム(Enterococcus faecium;VRE)は院内感染の主要な原因の 1 つであるが、これは VRE が患者間で伝播し病院環境で生き残る能力によるものである。
目的
長い歴史を持つ胸部心臓病院が個室の割合が 100%に近い新施設へ移転する際の VRE による環境汚染率、VRE 感染率または保菌率に及ぼす影響を調査すること。
方法
2019 年 4 月から 11 月の 5 つの時点(旧病棟で 1 回、新病棟で 4 回)で、VRE の前向き環境サーベイランスを実施した。病院移転前と移転後の 1 年間について、VRE 感染/保菌の発生率比(IRR)を決定し、近隣病院と比較した。
結果
旧病棟では、初回の環境スクリーニングで 29%が VRE 陽性と確認された。新病棟での次の 4 回のスクリーニングによって、陽性率の有意な低下が示された(1 ~ 6%、P < 0.0001)。新病棟における VRE 感染率/保菌率は旧病棟と比較して半減し(IRR 0.56、95%信頼区間 0.38 ~ 0.84)、同じ期間の近隣病院での上昇(1.62、1.17 ~ 2.27)と対照的であった。環境分離株のゲノム解析は、新病院での伝播の減少と一致していた。
結論
個室の使用は VRE による環境汚染の減少、伝播の減少および臨床サンプルからの VRE の分離の減少に関連していた。医療関連感染の減少における個室病院の費用対効果は、新たに発生するパンデミックおよび増加する抗菌薬耐性の脅威への対応コストという観点から再評価すべきである。
監訳者コメント:
個室病院では VRE の環境汚染が減少し、ゲノム解析により伝播の減少が明らかになった。日本の医療施設においても、もっと個室が使用できれば感染対策は実施しやすくなるだろう。
