Financial impact of nosocomial infections on surgical patients in an eastern Chinese hospital: a propensity score matching study

中国の Zhejiang Taizhou Hospital で実施した本研究は、院内感染が外科患者に及ぼす経済的影響を検討することを目的とした。2022 年 1 月から 9月の 9 か月間に、傾向スコアマッチングを用いた後向き症例対照研究を実施した。本研究は、院内感染した外科患者 729 例と感染のなかったマッチさせた対照 2,187 例を対象とした。医療費、入院期間、総経済的負担を 2 群間で比較した。外科患者における院内感染率は 2.66％であった。入院費用の中央値は、院内感染した患者で 8,220 米ドルであったのに対し、対照では 3,294 米ドルであった。院内感染に起因する追加の医療費は全体で 4,908 米ドルに達した。院内感染症例と対照との間で、総入院費用、看護費用、薬剤費、処置費、材料費、検査費、輸血費に顕著な中央値の差が認められた。各年齢層において、院内感染した患者の医療費は、対照の 2 倍を超えていた。さらに、院内感染した外科患者の入院期間は、対照と比較して平均で 13 日間長かった。これらの結果から、患者と医療制度の経済的負担を軽減するためには、病院内での有効な感染制御策の実施が重要であることが強調される。

監訳者コメント：

外科患者の院内感染による経済的影響に関する研究である。特に比較にどのようなパラメーター（入院期間、看護費用、薬剤費、処置費など）を用いているかは我々が同様の評価を行う上でも参考になるだろう。
