自己報告による手指衛生遵守率の妥当性および観察された遵守率の行動決定因子の妥当性：オランダの病院 9 施設における COM-B 手指衛生質問票の実施★

背景
手指衛生遵守率は行動決定因子による影響を受ける可能性があるが、このことに関する知識は少ない。「能力、機会、動機づけ‐行動（Capability, Opportunity, Motivation- Behaviour；COM-B）」手指衛生質問票は、自己報告による行動決定因子および自己報告による手指衛生遵守率について洞察を得る目的で、Lydonらによって開発された。
目的
COM-B 質問票を用いた自己報告による手指衛生遵守率の妥当性を明らかにすること、また観察された手指衛生遵守率に対して、自己報告による行動決定因子が及ぼす影響について、環境決定因子を考慮に入れて検討すること。
方法
本研究は横断研究であり、2019 年 9 月から 11 月にオランダの病院 9 施設で実施した。医療従事者に COM-B 質問票に回答してもらい、手指衛生の直接観察を実施した。さらに、環境決定因子（労働量、病棟のカテゴリー、病院のタイプ、および病棟のインフラ）に関する情報を収集した。自己報告による手指衛生遵守率の妥当性を、級内相関係数（ICC）を用いて判定した。単変量および多変量回帰分析を実施して、行動および環境決定因子と、観察された手指衛生遵守率との関係を検討した。
結果
ICC では、自己報告による手指衛生遵守率と観察された手指衛生遵守率との間に関連は示されなかった（ICC 0.04、95％CI －0.14 ～ 0.21）。単変量解析では、病棟のカテゴリー、機会、および動機づけのサブスケールには、観察された手指衛生遵守率との間に有意な関連が認められた。多変量回帰分析では、病棟のカテゴリーと動機づけのサブスケールのみが依然として有意であった。
結論
自己報告による手指衛生遵守率は、手指衛生の直接観察の代替指標として妥当ではない。動機づけ（行動決定因子）は手指衛生遵守率と有意に関連した一方、観察された手指衛生遵守率に影響を及ぼした環境因子ではほとんどなかった。手指衛生介入のさらなる向上において、医療従事者の内的な動機づけに対してこれまで以上に注意を向けるべきである。
監訳者コメント：
本研究結果により、COM-B 質問票による自己報告の手指衛生遵守率は、直接観察の手指衛生遵守率に取って代わることができないことが示唆された。
