様々な臨床環境の病院職員の呼吸器防護具に関するルーチンの（非アウトブレイク時の）行動の理解：COVID-19 後の未来のための教訓★★

背景
新型コロナウイルス感染症（COVID-19）パンデミックは、顔面（呼吸器および眼）の防護具の重要性に注目を集めた。非アウトブレイク状況で顔面防護具を最適に使用することによって、救急部の臨床医のような最前線の職員が、感染症のアウトブレイク時に増加する必要性と求められる技術に、より迅速・安全に適応できる可能性がある。
方法
COVID-19 より前に、呼吸器感染症からの保護を目的とする顔面防護具の使用にまつわる医療従事者の姿勢、信念および知識を明らかにするためにデザインされた調査を、オーストラリア・シドニーの呼吸器病棟 1 棟、成人救急部 1 カ所、小児救急部 1 カ所の職員に配布した。
結果
本調査によって、呼吸器病棟と救急部の違い、専門家集団間の違いが明らかになった。救急部の職員、特に小児科臨床医は、病棟職員よりもルーチンの治療中に顔面防護具を適切に使用することが少なかった。医療職員は感染予防・制御方針外で勤務する傾向が強かった。
考察
呼吸器症状を有する患者を治療する際の顔面防護具の安全使用の最適な遵守に対して、多忙で比較的混沌とした救急部の環境が独特の課題を突き付けている。
結論
パンデミックの教訓を踏まえて、非アウトブレイク状況時に顔面防護具の使用の遵守を改善するために、救急部の環境特有の感染予防・制御のニーズに対応するのは時宜にかなっている。
監訳者コメント：
インフルエンザの鼻咽頭スワブ採取時やインフルエンザ様症状のある患者が 1 m 以内に居るときになど、眼部保護具を必ず着用するのはどちらの場合でもわずか 5％と低く、眼部保護への意識が低い。一方 N95 マスクは、結核患者の部屋に入室するときには 92％が着用、空気感染対応時には 78％が着用するとの高い着用率だが、鼻咽頭スワブ採取時の N95 着用率は 16％と低くなる。救急外来での防護具の対応については、施設差や個人差が極めて大きいことがわかっている。本研究は 2017 年から 2019 年の新型コロナ前であり、新型コロナのパンデミックと感染経路がエアロゾル感染であることが明らかとなった現在ははるかにその遵守率は改善していることが予測される。
