麻酔導入における WHO『手指衛生 5 つのタイミング』：ビデオに基づく解析により新たなシステムの課題と設計機会が明らかに★

背景
麻酔導入はペースが速く複雑な活動であり、高密度に手指と表面の接触が生じる。手指衛生遵守率は低いことが報告されており、そのため連続患者間で気付かれることなく病原体が伝播する可能性がある。
目的
世界保健機関（WHO）が推進する『手指衛生5つのタイミング』コンセプトの、麻酔導入ワークフローへの適合度を検討する。
方法
5 例の麻酔導入のビデオ記録を、WHO 手指衛生観察法に従って、関与したすべての麻酔提供者の手指と表面の接触それぞれを考慮に入れ、解析した。二項ロジスティック回帰分析を用いて、不遵守のリスク因子、すなわち職種、性別、業務役割、手袋、物を手に持つこと、チームの規模、手指消毒のタイミングを検討した。さらに、全ビデオの半分を、提供者の自己接触の定量的および定性的解析のために再コード化した。
結果
全体で手指衛生機会 2,240 回に対して取られた手指衛生行動は 105 回（4.7％）であった。薬剤投与の役割（オッズ比［OR］2.2）、上級医師であること（OR 2.1）、手袋を着けること（OR 2.6）および外すこと（OR 3.6）が、より高い手指衛生遵守率と関連した。特に、手指衛生機会全体の 47.2％は、自己接触行動によって生じていた。提供者の衣服、顔、患者の皮膚が最も高頻度に接触された表面であった。
結論
高密度の手指と表面の接触、高い認知的負荷、長時間の手袋使用、運ばれる可動性の物、自己接触、個人的行動パターンが、不遵守の原因となる可能性があった。指定された物や提供者の衣服を患者区域に導入することを含め、これらの結果に基づき目的に合うように設計された手指衛生コンセプトによって、手指衛生の遵守と微生物学的安全性が向上する可能性がある。
監訳者コメント：
「全体で手指衛生機会 2,240 回に対して取られた手指衛生行動は 105 回（4.7％）であった。」いうことにより、このタイミングでは麻酔科業務において手指衛生活動の指標の可視化について再考する必要があろう。
