Influence of environmental temperature and heatwaves on surgical site infection after hip and knee arthroplasty: a nationwide study

背景

これまでの研究では、夏季または高温度下で実施された手術後に手術部位感染症（SSI）の発生率が高いことが報告されている。しかし、詳細な気候データを用いて股関節および膝関節置換術後の SSI リスクを評価した研究はなく、また猛暑の役割について特に検討した研究はない。

目的

股関節および膝関節置換術後の SSI 発生率に、高い環境温度および猛暑が及ぼす影響を評価すること。

方法

2013 年 1 月から 2019 年 9 月にかけて、Swiss SSI サーベイランスの参加病院で実施された股関節および膝関節置換術に関するデータを、参加病院の近傍に位置する気象台から得たデータにリンク付けした。温度、猛暑と SSI との関連を、患者レベルにフィットさせた混合効果ロジスティック回帰モデルを用いて検討した。Poisson 混合モデルを、暦年および歴月の両方にフィットさせて、SSI 発生率の経時的な動向を検討した。

結果

病院 122 施設で実施された手術 116,981 件を組み入れた。有意に高い SSI 発生率が、夏季（発生率比 1.39, 95％信頼区間［CI］1.20 ～ 1.60、P ＜ 0.001、秋季を基準）または平均温度 20°C 超の歴月（基準 5 ～ 10°C、オッズ比 1.59、95％CI 1.27 ～ 1.98、P ＜ 0.001）に実施された手術で認められた。猛暑時には、SSI 発生率に有意でないがわずかな上昇が認められた（1.44％対 1.01％、P ＝ 0.2）。

結論

股関節および膝関節置換術後の SSI 発生率は、環境温度が高い場合に上昇するようである。猛暑が SSI のリスクを高めるのか、また高めるならどの程度であるのかを確立するために、温度に大きなばらつきのある地域を含めた研究が必要である。
