Epidemiology of occupational exposure to blood-borne viruses, postexposure prophylaxis and seroconversion over 10 years among healthcare workers

背景

医療従事者は、職業的血液曝露による血液媒介病原体を原因とする感染症のリスクが高い。早期の報告、検出および曝露後予防が、感染症予防において有用となる。

目的

職業的血液曝露の発生率、関連する疫学的特徴、曝露後予防の完了率、曝露後予防中止の時点および理由、ならびに 10 年間に報告された血清陽性の発現率について検討すること。

方法

本後向き研究では、2012 年 1 月から 2021 年 12 月に韓国の 3 次病院 1 施設のスタッフを対象に、職業的血液曝露を認め、曝露後予防管理を受けた 1,086 例について解析を行った。

結果

職業的血液曝露の平均発生率は、100 床当たり 7.82、および医療従事者 100 例当たり 3.0 であった。職業的血液曝露を受けた 1,086 例中、633 例（58.3％）が曝露後予防を必要とし、453 例（41.7％）は必要としなかった。職業的血液曝露後に、曝露後予防を受けた医療従事者の70.1％（633 例中 444 例）で追跡が完了し、29.9％（633 例中 189 例）で曝露後予防の追跡が中止された（P ＜ 0.001）。曝露後予防の完了率には、性別（P ＝ 0.024）、職種（P ＜ 0.001）および曝露頻度（P ＜ 0.001）により有意差が認められた。曝露後予防を完了した医療従事者 444 例中、B 型肝炎ウイルス、C 型肝炎ウイルス、ヒト免疫不全ウイルスまたは梅毒トレポネーマ（梅毒）の血清陽性を認めた例はなかった。

結論

本研究の結果は、職業的血液曝露後の医療従事者に対するフォローアップケアを改善する必要があることを示している。医療施設における教育が必要であり、さらに、全国サーベイランスシステムを確立することで、医療従事者が先を見越した曝露後予防を受けられるように、またそのフォローアップ受診を完了できるようにすることが必要である。
