Risk of microbial transmission by reusing gloves after alcohol-based hand hygiene

背景

手袋の消毒は医療従事者の作業負荷を減少させ、環境を保護し、経済的利益をもたらす可能性がある。したがって、手袋を装着した手の手指衛生の安全性は重要な問題である。

目的

診療現場において、手袋を装着した状態または装着していない状態で、複数回の患者との接触後の残存微生物を比較することによって、微生物伝播リスクを評価することを目的とした。

方法

手袋（一重または二重手袋）を装着した 2 名と素手の 1 名の調査者は、患者のラウンドを行い、その後、擦式アルコール製剤を使用した。ラウンドの前後に手のインプリントを採取し、培養した。手袋を装着した手と素手のコロニー形成単位（cfu）数を比較し、手の部位ごとに半定量的にコロニー分布を評価した。

結果と結論

10 回のラウンド後に計 108 のインプリントを採取した。cfu 数の中央値は、一重および二重手袋を装着した手のほうが素手よりも有意に高かった（9.00 対 3.50、P ＝ 0.028）。一重および二重手袋を装着した手の cfu 数は、接触後のほうが接触前よりも高かった（それぞれ P ＝ 0.044、P ＝ 0.001）。1 回のラウンド後、一組の二重手袋にカルバペネム耐性アシネトバクター・バウマニー（Acinetobacter baumannii）が確認されたが、そのラウンドには抗菌薬感受性の結果が同一である同じ菌を有する患者が含まれた。指と手首の部位において、手袋を装着した手のコロニーが増殖する区画の割合の平均値は、素手よりも有意に高かった（それぞれ P ＝ 0.019、P ＝ 0.049）。擦式アルコール製剤を使用した後であっても、素手と比較すると、手袋の再利用は残存微生物のコロニーを増加させ、多剤耐性菌の伝播の可能性を高めた。

監訳者コメント：

手袋装着時の擦式アルコール製剤による手指衛生後の手袋の再利用は医療従事者の作業負荷を軽減し、コストの削減をもたらす可能性があるものの、素手による手指衛生と比べると多剤耐性菌の伝播のリスクが高いことが改めて認識できる。
