背景

抗菌薬耐性菌の出現の増加に伴い、経済および衛生上の課題が増大している。電子カルテシステムの利用の増加ならびにコンピューター化による分析技術の改善により、入院時の抗菌薬耐性菌保菌者を特定する臨床予測ツールの開発を通して、抗菌薬耐性菌の拡大を低減する機会が得られる。

目的

メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus:MRSA)およびカルバペネマーゼ産生菌の既存の臨床予測ツール、その予測性能、およびこれらのツールに用いるリスク因子を明らかにすること。

方法

CHARMS チェックリストを遵守した。Medline、EMBASE、Cochrane SR、CRD データベース(DARE、NHS EED)、CINAHL、Web of Science を用いて、データベースの検索開始から 2021 年 7 月 26 日まで検索した。フルテキストの論文を独立して評価し、Prediction Model Risk of Bias Assessment Tool を用いて質的評価を実施した。

結果

計 3,809 件の抄録を特定し、研究 22 報を対象とした。これらの研究において、もっとも一般的な予測ツールはリスクスコアモデルであった(16 ツール)。抗菌薬耐性菌保菌のリスク因子として、入院歴、最近の抗菌薬曝露、年齢、性別がもっとも多かった。予測ツールは感受性および特異性を用いて評価されることが多く、感受性および特異性の範囲はそれぞれ、MRSA では15 ~ 100%、46 ~ 98.6%、カルバペネマーゼ産生菌では 30 ~ 81.3%、79.8 ~ 99.9%であった。

結論

ゴールドスタンダードとされる抗菌薬耐性菌の予測ツールはない。しかし、耐性菌の早期検出のための高性能臨床予測ツールによる重要なリスク因子の特定法がある。リスクスコアモデルは使用と解釈が容易であるが、近年の機械学習技術の向上により、電子カルテに蓄積されたデータによって頑健性の高いモデルが開発される可能性がある。

監訳者コメント

“臨床予測ツール”とは、いかにも今風のタイトルである。こうした「高性能臨床予測ツール」を解析し、その妥当性を評価することが今後は必要だろう。入院歴、最近の抗菌薬曝露、年齢、性別は何れも古典的な耐性菌の保菌者のスクリーニングに使用される項目であるが関連各項目重み付けなどの評価が必要であろう。
