背景

SARS-CoV-2 の院内伝播は制御しにくいことが判明しており、期間を通して医療関連感染症の問題を伴う。

目的

拡散の封じ込めが必要な感染症において院内伝播の原因となる要因を理解すること。

方法

3 次紹介施設の職員および患者 56 例の 31 日間に及ぶ新型コロナウイルス感染症(COVID-19)アウトブレイクが認められ、全ゲノムシークエンシング(WGS)により、その施設と病院以外で少なくともさらに 29 例が特定された。

結果

職員-職員間、職員-患者間、患者-職員間で伝播が認められ、全国的に推奨される制御策の実施、優れた換気、個人防護具使用にもかかわらず、3 次紹介施設のサービスが寸断した。発端患者は、間仕切りで厳重に隔離される病棟のベッド上に 10 時間だけとどまり、PCR 検査で最初の単一標的が低レベル(CT = 32)であったにもかかわらず、この患者から広範に拡散した。

結論

本研究は、SARS-CoV-2 が特定の環境下で、いかに効率的かつ迅速に拡散するかを浮き彫りにするものである。その時点の適所での感染制御策について疑問を提起するとともに、感度の低い迅速ラテラルフロー抗原検査を用いることにより、伝播性が確実に検出できるという根拠を疑問視する。さらに、アウトブレイクを理解した上で、影響の軽減と WGS の有用性を踏まえた早期介入の重要性を強調する。

監訳者コメント:

CT 値の高い(32)患者の短時間(10 時間)の滞在が契機になったクラスターの事例を全ゲノムシーケンスで分析した論文である。初発患者の感染性が低かったとしても、いったんそこから広がると二次感染、三次感染と、あっという間に大規模なクラスターに発展するのが SARS-CoV-2 のやっかいなところである。
