オランダの全国プログラムにおける中心静脈カテーテル関連血流感染症を予防するための介入バンドルの効果★

背景
中心静脈カテーテル(CVC)は中心ライン関連血流感染症(CRBSI)につながることがある。2009 年にオランダの病院で、CRBSI を予防するための6 項目からなるバンドルが導入された。
目的
本研究は、介入バンドルが CRBSI リスクに及ぼす効果を明らかにすることを目的とした。
方法
2009 年から 2019 年に、全国 CRBSI サーベイランスに参加している病院からデータを得た。バンドルへの遵守について、総(「全体」)バンドル(全 6 項目)および挿入バンドル(4 項目)および維持バンドル(1 日 2 回のチェック)として評価した。バンドルの全体および部分による影響を、マルチレベル Cox 回帰を用いて推定した。
結果
CRBSI サーベイランスの参加病院 66 施設中 56 施設(84.8%)で、1 ~ 9 年にわたり80%を超える CVC についてバンドルへの年間(不)遵守率が記録されていた。これらの病院 56 施設において、CRBSI 発生率は 1,000 CVC 日あたり 4.0 件から 1.6 件に低下していた。集中治療室では、遵守率は CRBSI リスクと関連していなかった(全バンドル、挿入バンドル、維持バンドルのハザード比[HR]は、それぞれ 1.14[95%信頼区間{CI}0.80 ~ 1.64)、1.05[0.56 ~ 1.95]、1.13[0.79 ~ 1.62])。集中治療室以外では、全バンドルへの遵守率との有意ではない関連(HR 1.36[0.96 ~ 1.93])は、リスク低下と関連した挿入バンドル(HR 0.50[0.30 ~ 0.85])およびリスク増加と関連した維持バンドル(HR 1.68[1.19 ~ 2.36])による逆の効果の結果であった。
結論
介入バンドルを導入する全国プログラムの実施後、CRBSI 発生率は有意に低下した。集中治療室では、バンドルの遵守率は CRBSI リスクと関連しなかったが、集中治療室以外における挿入バンドルの遵守率改善は CRBSI リスクの低下をもたらした。
監訳者コメント:
オランダの病院で CRBSI 予防バンドル(CV 挿入に関する 4 項目:手指衛生、挿入中の予防策、皮膚消毒、挿入部位の選択、維持管理に関する 2 項目:CV 適応、挿入部位のチェック)を導入したところ、CRBSI 発生率は減少した(特に ICU 以外で)。バンドルの各項目が CRBSI 予防策を考える上で参考になる。
