ドアハンドル消毒システムを使用して医療環境における媒介物上の微生物量に伴うリスクを低減する★

背景
多剤耐性菌を含む病原性微生物は、表面上で長期間にわたり生存することができる。数多くの研究により、ドアハンドルなどの汚染された手指接触部位が、その後の患者への伝播という重大なリスクをもたらす。
目的
ダブリンの国立整形外科病院の 2 病棟からなる多忙なリハビリテーション病棟において、頻繁に使用されるドア10 カ所のハンドル上の微生物量を、ドアハンドル消毒システムを設置した場合と設置しない場合について比較すること。
方法
ドアハンドル消毒システム(Handle Hygiene®)は、噴霧ポンプ(非エアロゾル)を利用して、ドアが使用されるたびにドアハンドルに対して一定量の消毒薬を自動的に供給する。頻繁に使用されるドア設置個所のハンドル上の微生物量を 16 週間にわたりモニタリングし、ドアハンドル消毒システムを設置した場合と設置しない場合について微生物量を比較した。本研究では、2 種類の消毒薬、Steri-7(広域スペクトラム消毒薬)と Dew(次亜塩素酸)を用いた。
結果
2.5 cfu/cm2 以下の微生物量が記録されたのは、ドアハンドル消毒システムが設置されていたドアから採取されたサンプルの 93%であり、サンプルの 66%では微生物が回収されなかった。2.5 cfu/cm2 超の微生物量が記録されたドアでは、ドアハンドル消毒システムにより、次にサンプルが採取された時にはこの濃度が検出不能なレベルまで減少したのに対し、同システムが設置されていなかったドアでは数日後でも微生物が検出された。
結論
ドアハンドル消毒システムは、追加費用が最小限で可能な自動化ソリューションとして、頻繁に触れられるドアハンドルに対して微生物量を低減するための効果的な解決法となる。
監訳者コメント:
医療従事者は手指衛生の遵守により微生物の患者への伝播を低減することができるが、患者の手指衛生は重要であるものの遵守は難しい。患者同士が使用するドアハンドルでは医療環境からの伝播のリスクになり、こうしたドアハンドル消毒システムが簡便で、安価で、安全性の高いものであれば、リスクを低減する一つの方法にはなるだろう。
