韓国における抗菌薬適正使用支援に対する看護師の態度★

背景
抗菌薬耐性についての世界的懸念により、診療の安全性と質を改善するための抗菌薬適正使用支援プログラムの作成が急務である。看護師は抗菌薬適正使用支援に重要な役割を担っている。
目的
抗菌薬適正使用支援に対する韓国の看護師の態度を評価し、組織的要因による抗菌薬の使用および耐性に対する看護師の態度の違いを比較すること。
方法
今回の記述的横断研究では、看護師 1,142 名のオンライン調査データを分析した。分散分析を用いて、組織的要因による看護師の態度を比較した。
結果
看護師の約半数が抗菌薬適正使用支援の用語をよく知っており、知識が豊富であった。回答者の大部分が、抗菌薬に関する十分な知識は必須であること、医療施設には抗菌薬適正使用支援プログラムが重要であること、抗菌薬使用を改善するための介入に看護師が関与すべきであることに同意した。看護師は感染症患者のケアについて疑問が生じた場合、同僚の看護師またはインターネットリソースに頼ることが多い。大規模病院で勤務する看護師または感染症専門家にアクセスしやすい看護師は、抗菌薬の使用と耐性について洞察力がより高かった。
結論
抗菌薬適正使用および抗菌薬耐性に関して、病院は看護師向けの教育プログラムを提供する必要がある。抗菌薬適正使用支援プログラムおよび感染症専門家との連携体制を含む包括的なアプローチにより、看護師は抗菌薬使用に関する科学的エビデンスをさらに入手しやすくなるであろう。リソースが限られた医療環境における抗菌薬適正使用支援プログラムを構築するために、全国レベルでのアプローチを行うべきである。
監訳者コメント：
抗菌薬適正使用支援に対する韓国の看護師の態度を評価した論文である。日本でも、ASP チームにおいて看護師が活躍しており、同様の結果、さらには課題が考えられるものと思われた。
