歯学部学生の針刺し切創・鋭利物損傷の世界的な有病率、リスク因子および報告の実践:システマティックレビューとメタアナリシス

背景
歯学部学生は、不十分な訓練が原因で針刺し切創を起こしやすい。しかし、歯学部学生の針刺し切創の世界的な有病率は不明である。
目的
針刺し切創に関して、歯学部学生におけるプールした有病率、疫学的特性およびリスク因子を明らかにすること。
方法
Preferred Reporting Items for Systematic Reviews and Meta-Analysis(PRISMA)ガイドラインに従って、システマティックレビューを実施した。本レビューのプロトコルは International Prospective Register of Systematic Reviews に登録された(CRD42022312778)。PubMed、Scopus、Web of Science、Embase、OVID、EBSCO のデータベースから適格な研究を特定した。プールした有病率を推定するために変量効果モデルによるメタアナリシスを実施し、研究間の不均一性を検討するためにメタ回帰を行った。
結果
15 か国の計 25 件の研究が組み入れ基準に合致した。歯学部学生における針刺し切創のプールした有病率の推定値は 44%(95%信頼区間 38 ~ 51%)であった。局所麻酔、歯磨きまたは歯石除去、廃棄物処理が針刺し切創の最も高いリスクに関連していた。大部分の研究で針刺し切創の過少報告が認められた。歯学部の学生は、曝露後の対応に関する知識が不十分であった。
結論
歯学部の学生は針刺し切創への曝露の有病率が高く、報告率は低かった。知識が不十分であることによって、針刺し切創への曝露の確率が高くなる可能性がある。
監訳者コメント:
監訳者も歯科領域の針刺し・切創、粘膜曝露事故の対応や教育に携わっているが、歯学部学生との接点はない。歯科領域においても、学部教育における感染防止教育が重要であり、そこへのアプローチが不十分であることを認識させられた。
