オランダとベルギーの国境地帯にある病院 9 施設における抗菌薬の責任ある使用のための一般的な質的指標の実践試験★

背景
抗菌薬の責任ある使用について評価するために、以前に入院患者の質的指標を作成した。
目的
病院環境における入院患者の質的指標の実践試験。
方法
本研究は、i-4-1-Health プロジェクトの一環として Infection Risk Scan による点有病率調査（PPS）を実施するオランダ‐ベルギー国境の病院ネットワーク内で実施した。DRIVE-AB プロジェクトにおける 51 の入院患者の質的指標のうち 20（構造の質的指標 13、プロセスの質的指標 7）について試験を行った。病院の微生物学者に送られたウェブベースの質問票により入院患者における構造の質的指標のデータを得た。2018 年 10 月から12 月の PPS のデータを用いて、プロセスの質的指標の遂行スコアを算出した。
結果
病院 9 施設が参加した。入院患者における構造の質的指標に関して：ガイドラインの微生物学的研究の推奨および制限された抗菌薬の認可システム使用において遂行スコアがもっとも低かった。さらに、ほとんどの病院で、一部の抗菌薬が不足しているため在庫がないことが報告された。入院患者におけるプロセスの質的指標に関して：抗菌薬全身投与の処方数 697 件を用いて遂行スコアを算出した。医療ファイルへの抗菌薬投与計画の記録でスコアがもっとも低かった（58.8％）。ガイドライン遵守に関する入院患者の質的指標の遂行スコアは、抗菌薬レジメンの異なる状況（期間、選択、経路、タイミング）において 74.1％～ 82.3％と差がみられた。
結論
入院患者の質的指標に関する今回の多施設共同実践試験により、抗菌薬治療での構造およびプロセスの両側面において、適正使用支援の取り組みの改善ターゲット（制限された抗菌薬の認可システム、抗菌薬投与計画の記録）が特定された。これらの結果は、今後の PPS の研究デザインと、入院患者の質的指標の評価手段特性に関するより詳細な評価を導くであろう。
監訳者コメント：
本研究は、オランダ、ベルギーの病院で行われた病院における入院患者 1,551 例の質的指標の実践試験である。用いられた指標は、病院の抗菌薬適正使用に対する客観的評価に使用できるのでは、と思われる。
