アフリカの入院患者における抗菌薬適正使用支援介入が抗菌薬処方の改善に及ぼす影響：システマティックレビューおよびメタアナリシス

抗菌薬使用の改善を図る抗菌薬適正使用支援介入がアフリカで実施されているが、その影響は明らかにされていない。本レビューの目的は、入院患者への抗菌薬処方を改善する介入の有効性を推定することである。PubMed、Embase、African Journals Online、Google Scholar 掲載の研究を、2010 年 1 月から 2022 年 7 月まで系統的に検索した。対象とする研究は、アフリカの入院患者における抗菌薬適正使用支援介入が、着目するアウトカムに及ぼす影響を報告している場合とした。バイアスリスクを、Cochrane Effective Practice and Organization of Care のガイドラインおよび National Heart, Lung and Blood Institute のツールにより評価した。結果の概要を表に示し、ランダム効果モデルを用いてメタアナリシスを実施した。計 28 報の研究を対象とし、非対照の前後比較研究が 89％を占めた。使用された介入のほとんどが多面的であり、有効であることが明らかにされ、このことは遵守率の増加、抗菌薬の使用と費用の減少、死亡率と入院期間（LOS）のわずかな減少または差がないことにより立証された。メタアナリシスにより、死亡の相対リスク 0.82（95％信頼区間［CI］0.70 ～ 0.97]、LOS の標準平均差－0.30（95％CI －0.41 ～ －0.19）が得られた。総じて、大部分の微生物に対する抵抗性の低下がみられた。アフリカでの抗菌薬適正使用支援の研究数が増加しているにもかかわらず、抗菌薬適正使用支援の研究において質的なデザイン特性の大部分が不十分である。結論として、抗菌薬適正使用支援介入は有効である可能性が高いが、妥当性に要する質的なデザイン特性を研究デザインに組み入れて、実践に情報を提供する取り組みがさらに必要である。
