背景

黄色ブドウ球菌(Staphylococcus aureus)は、かなりの死亡および医療制度に対する負担増加と関連する。メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)感染症については、その影響に関する信頼できる推定を行った研究が、メチシリン感受性黄色ブドウ球菌(meticillin-sensitive Staphylococcus aureus;MSSA)感染症と比べて比較的少ない。

 

目的

MRSA 感染症と MSSA 感染症の患者を比較して、院内死亡、入院期間、および医療提供コストにおける差を明らかにすること、ならびにブドウ球菌感染症の原因としての MRSA の予測因子を明らかにすること。

 

方法

分析的後向き縦断研究を、10 年間にわたりブドウ球菌感染症の診断とともにオーストラリアの 1 医療圏の病院に入院した成人の匿名化されたリンクデータを用いて実施した。主要転帰評価項目は 30 日院内死亡率とした。副次的転帰評価項目は、初回入院から 1 年以内における、総入院期間、全死因院内死亡率、および入院コストなどとした。

 

結果

30 日、100 日、および 365 日院内死亡率の推定値は、MRSA 感染症患者のほうが有意に高かった。初回入院から 1 年以内の、追加の因子で補正した MRSA 感染症による平均追加コストは 5,988 ドル、患者当たりの追加入院日は 4 日であった。MRSA 感染症の主要予測因子は、初回入院日、併存症スコア高値、大きな社会経済的不利、救急部以外を介した入院、高齢、および初回入院前 28 日以内の入院歴であった。

 

結論

MRSA 感染症は MSSA 感染症と比較して、院内死亡率高値、コストの増加、および入院期間の延長と関連した。患者および医療制度に対する MRSA 感染症による負担の増加を軽減するための取り組みが必要である。

監訳者コメント

MRSA 感染症は、MSSA 感染症と比較した場合には、30 日死亡率や全死亡率、入院期間の延長があることはすでに WHO により報告されている。これまでは単施設での報告が多いなか、本論文は 40 万人以上の人口をもつ医療圏からの後方視的にデータを収集しており、オーストラリアでの状況を反映するものと推察される。これらの結果から抗菌薬適正使用と手指衛生遵守率向上を含む感染対策の再徹底が必要である。
