Improvement and standardization of disinfection in hospital theatre with ultraviolet-C technology

背景

手術室の交差汚染リスクを防ぐために、各手術の間の消毒の段階は重要である。

目的

短波長紫外線（UVC）装置の適用前（T₀）および適用後（T₁）に、消毒の各段階における微生物汚染の差を評価すること。

方法

2019 年 12 月から 2020 年 8 月までクリニック1 施設において横断研究を実施した。ベッドサイドあたりの UVC 装置の適用 3 ～ 5 分に従い、T₀ および T₁ での微生物存在の減少を評価するために、3 通りの消毒の段階（手術後消毒なし、各手術間の洗浄後、最終洗浄後）を比較した。計 260 のペトリ皿を予備段階とその後の確率モデル主導型実験に分けて、3 つの手術室で使用し、コロニー形成単位（cfu）を測定した。UVC 曝露時間を設定するために、予備段階で Mann-Whitney 検定を実施した。確率モデルを用いて、記述統計量と割合、log₁₀ 減少を測定した。消毒の段階と各手術室を組み合わせて T₀ と T₁ 間の 95％統計学的有意差を検証するために、反復測定の多変量分散分析（MANOVA）を実施した。

結果

Mann-Whitney 検定では、UVC 曝露時間 3 分と 5 分との間で cfu の差は認められなかった。MANOVA 検定では、すでに適用した消毒の段階に関係なく、T₀、T₁ の消毒の段階の間で cfu 減少に有意差はみられず、cfu の平均低下は 72％（95％信頼区間 61.7 ～ 84.9）であった。

結論

最初のすべての条件において、UVC 装置は環境消毒を改善した。UVC 装置は標準化され、既存の古典的な消毒法とともに環境衛生のレベルを標準化すると思われる。

手術室における紫外線照射による微生物の減少を示した研究。紫外線照射は手術室の清浄度を高めるのにも有用であるが、どのタイミングで照射するか運用を決めることも重要である。
