Environmental detection of SARS-CoV-2 in hospital rooms in different wards of a university hospital

背景

重症急性呼吸器症候群コロナウイルス-2（SARS-CoV-2）の伝播は、感染者との直接的、間接的、または濃厚な接触を介して生じ得る。しかし、環境汚染の程度については知られていない。患者の症状と、SARS-CoV-2 の環境内排出との関係の性質は、依然として不明である。本研究の目的は、患者の新型コロナウイルス感染症（COVID-19）の状態と環境汚染との関係について評価することであった。

方法

2020 年 5 月から 11 月の間に、臨床的または生物学的に COVID-19 と診断された患者のコホートにおいて、発症の 7 日後に病室を消毒した前後に環境スワブを採取した。13 部屋について、部屋当たり 12 表面から採取した。サンプルの分析を、SARS-CoV-2 検出用の逆転写ポリメラーゼ連鎖反応法（RT-PCR）（SARS-CoV-2 R-Gene［biomérieux社、Marcy l’Etoile、France］）により実施した。臨床データ（発症日、症状、RT-PCR 結果）を、臨床ソフトウェアから収集した。

結果

5 つの医療ユニットを研究対象とした。13 室で採取したサンプル 156 個のうち、5 部屋（38.5％）で SARS-CoV-2 陽性サンプル 11 個が得られた。これらの陽性サンプルは、8 つの異なる表面で検出された。SARS-CoV-2 の検出には、患者の年齢（P ＝ 1）または患者の症状（P ＝ 0.3）との関連は認められなかった。

結論

COVID-19 感染時のウイルス排出は、症状の存在、患者の年齢、および患者の検査におけるCt値（サイクル閾値）低値とは関係ないようであった。本研究は、発症から 7 日後まで環境内に SARS-CoV-2 が排出されるというエビデンスを支持している。これにより、COVID-19 患者に対するルーチンのケアとして、接触隔離、手指衛生の厳格な遵守、個人防護具の正確な使用、ならびに病室の消毒が必要であることが強く示されている。

監訳者コメント：

フランスの大学病院で、COVID-19 の患者の病室で環境をスワブし、SARS-CoV-2 が RT-PCR で検出されるのか調査した研究。サンプルからは発症後 7 日の環境から SARS-CoV-2 が陽性となるものがみられたことから、COVID-19 に対して標準予防策や接触予防策を厳守する必要性が強調されている。しかしながら、オミクロン株が検出される前の研究であること、RT-PCR の Ct 値の記載があるものは値が高く、またウイルス培養は行われていない点で、感染性の有無については不明であることなど、制限の多い研究であり結果の解釈には注意が必要である。
