Incidence and risk factors for hospital-acquired cholecystitis

背景

急性胆嚢炎は病院環境内外で発生するが、院内発症胆嚢炎の臨床的特性についてはほとんど知られていない。

目的

3 次大学病院における院内発症胆嚢炎の臨床的特性を検討すること。

方法

本後向きコホート研究では、2018 年 1 月から 2021 年 12 月 までの間の入院時に胆嚢炎と胆管炎のいずれも伴わない胆石が認められた入院患者を対象とした。院内発症胆嚢炎の罹患者と非罹患者間で比較するために、多変量ロジスティック回帰分析を用いた。

結果

本研究において患者計 890 例が選択基準を満たし、評価を受けた。研究期間に 41 例（4.6％）が院内発症胆嚢炎を発症した。多変量ロジスティック回帰分析では、胆嚢炎または胆管炎の既往、1 日以上の絶食、胆嚢頸部における胆石が、院内発症胆嚢炎のリスク増加と独立して関連した。院内発症胆嚢炎は入院数週間後の発症率がもっとも高く、菌血症がみられたのは 4 例（9.8％）のみであった。

結論

入院患者において院内発症胆嚢炎は比較的よくみられる。医師は、特定のリスク因子のある有症状の入院患者での院内発症胆嚢炎の可能性を認識すべきである。

監訳者コメント：

入院時に胆石を有する患者のうち、①胆嚢炎または胆管炎の既往のある患者、②1 日以上の絶食を行う患者、③胆石が胆嚢頸部にある患者、では入院中の胆嚢炎発症に注意が必要という結果は感覚的にも納得感のあるものである。
