Nosocomial bloodstream infection pathogen Pantoea dispersa: a case report and literature review

Pantoea dispersaは、植物、土壌、水中によく認められるグラム陰性菌である。ヒト感染症では Pantoea属はまれな病原体である。乳児、術後患者、免疫能低下患者などが感染しやすい集団として知られている。胸腔穿刺後の P. dispersa による院内血流感染症の報告はこれまでなかった。胸内苦悶を呈する 72 歳の中国人女性が、血液培養でグラム陰性菌陽性であることが確認された。この微生物は、Vitek 2、マトリックス支援レーザー脱離イオン化飛行時間型質量分析法、16S rRNA 遺伝子シークエンス法により P. dispersa と同定された。治療のためにセフォペラゾン‐スルバクタム、イミペネムが使用されたが、患者は 4 日後に死亡した。われわれの知る限りでは、これは P. dispersa に起因する院内血流感染症の中国初の症例である。この病原体の潜在的な病原性について、本稿が臨床医のさらなる理解の一助となることを期待する。
