経直腸超音波ガイド下前立腺生検は、病的状態、死亡、再入院、医療費の増加を伴う、処置後の敗血症のリスク上昇に関連している。試験対象施設では、前立腺癌の臨床専門看護師によって、感染制御チームの指導の下に、処置後感染症の合併症に関する積極的サーベイランスが実施されている。新型コロナウイルス感染症（COVID-19）パンデミックに関連する緊急入院に対する病院業務を保つため、経直腸超音波ガイド下生検の業務は、前立腺癌の高リスク患者に対してのみを例外として、全国的に削減された。試験対象施設では、この変更によって局所麻酔下で外来患者に実施する経会陰前立腺生検への完全移行が促された。経会陰前立腺生検によって処置後敗血症のリスクが排除され、その結果として COVID-19 パンデミック中の前立腺癌診断業務を維持しながら、敗血症関連の入院が解消された。

監訳者コメント：

　経直腸超音波ガイド下前立腺生検は、針が直腸を通って前立腺を穿刺するという方法で理論的にも感染が起きやすいことが想像される。その点で、針が腸管を通過しない経会陰前立腺生検は感染リスクが低い。日本でも両方の術式が行われているが、それぞれメリット・デメリットがあるようだ。感染管理担当者としては、前立腺生検といっても複数の術式があることを理解すること、そして生検後の感染の有無についてサーベイランスを行うことが重要である。
