Fit testing and comfort evaluation of prophylactic dressing use for healthcare workers under N95/P2 respirators in one health service district in Australia

背景

本研究では、N95/P2 マスク使用下での予防的ドレッシング(シリコンフォーム、シリコンテープ、ハイドロコロイド)の使用について評価し、的確にフィットするドレッシングを明らかにした。

 

目的

本研究の目的は、オーストラリアのひとつの州の医療プロトコールを作成することであった。

 

方法

医療従事者 600 名を対象とした Respiratory Protection Programme の一環として、2021 年 8 月と 9 月にデータ収集を行った。データ収集にあたって、3 種類の予防的ドレッシングを用いた。5 種類のマスクを用いた。参加した医療従事者に、4 点リカートスケールにより快適度を評価してもらった。

 

結果

的確なフィット性が達成されたのは、マスクとドレッシングの組み合わせのうち 63.6%であった。もっとも成績が優れていたマスクとドレッシングの組み合わせは、Trident® マスクとドレッシング Mepilex® Lite シリコンフォーム(合格率 90.2%)であった。高い合格率が示されたのは、Trident® マスクと Mepilex® Border Lite + SofSicure シリコンテープ(79.1%)、3M™ 1860 マスクと Mepilex® Border Lite + SofSicure シリコンテープ(74%)、ならびに BSN orange duckbill マスクと Mepilex® Lite シリコンフォーム(69.8%)であった。もっとも合格率が低かったのは、BYD™ マスクと Mepilex® Border Lite + SofSicure シリコンテープ(合格率 25.9%)であった。関連についての未補正の χ2 検定では、ドレッシングの種類と転帰の間(P = 0.004)およびマスクの種類と転帰の間(P < 0.001)に有意な関連が示された。ほとんどの回答者(82%)が、ドレッシングとの組み合わせは非常に快適であると答えた。

 

結論

N95/P2 マスクとともに予防的ドレッシングを用いる場合、フィットテストを行う必要がある。本研究では、Trident® マスクがフィットテストで合格する確率がもっとも高かった一方、BYD™ マスクはその確率がもっとも低かった。Trident® マスクと Mepilex® Lite ドレッシングの組み合わせが最適であった。ほとんどの参加者は、マスクの下にドレッシングを用いると、快適さが増すと報告した。

監訳者コメント:

N95/P2 マスクを長時間使用することにより皮膚障害が起きることが言われており、その予防としてマスクが接触する部位へのドレッシングが推奨されている。一方で皮膚へのドレッシングはマスクの性能を低下させる可能性があり、ドレッシングをして N95/P2 マスクを使用する際には、ドレッシング状態でフィットテストを実施し、皮膚に対して快適かつ有効性の高い呼吸器防護を行うことが重要である。
