Using the theory of planned behaviour to explain hand hygiene among nurses in Hong Kong during COVID-19

背景

新型コロナウイルス感染症（COVID-19）パンデミックの発生により、感染症蔓延を制御するための人の行動の重要性が強調されている。感染症伝播の抑制に、手指衛生は費用対効果がもっとも高い方法の 1 つである。

目的

本研究の目的は、計画的行動理論を適用して、COVID-19 アウトブレイク時の香港の病院の看護師における手指衛生に関する信念や行動を分析することである。

方法

香港全域の公立病院に勤務する看護師を対象に、手指衛生行動に対する態度・主観的規範・行動の統制感を調査するオンライン質問票に回答する被験者を募集して、横断研究を実施し、手指衛生に関する信念と知識も調査した。

結果

看護師計 122 名（女性 73％）が本研究に参加した。今回の被験者において自己報告による手指衛生実施率は 81.93％であり、ほぼ 3 分の 2 が登録後の感染制御の研修に参加していた。結果により、主観的規範と行動の統制感が、意思による手指衛生行動と有意に正の相関を示すことが明らかになった。とはいえ、本研究において、態度は手指衛生の意思と行動に影響を及ぼさなかった。

結論

香港の公立病院に勤務する看護師において、COVID-19 アウトブレイク時の手指衛生行動を説明するうえで、計画的行動理論は有用かつ効果的なフレームワークを提供する。感染症伝播との継続的な戦いには、手指衛生改善のための継続的な取り組みが必要である。

未知の病原性を有する可能性の高い新興感染症病原体に曝露する可能性のある医療従事者であれば、一般的な手指衛生剤が病原体不活化に効果的であることを知らされれば遵守率の向上に十分寄与するインセンティブとなりうる。
