カテーテル関連血流感染症に関する洞察:カテーテルへの細菌定着と多剤耐性に関する前向き観察研究

背景
中心静脈カテーテル関連血流感染症(CRBSI)は、死亡率および医療費にかなりの影響を及ぼす公衆衛生上の大きな懸念の 1 つである。
目的
CRBSI の主要な原因菌、それらが中心静脈カテーテルに定着する能力および抗菌薬耐性プロファイルを明らかにするため、ポルトガル・リスボンにある 3 次病院 3 施設を登録した 3 年間の観察研究をデザインした。
方法
CRBSI の原因菌を、Vitek 2 によって同定した。最も有病率が高い原因菌による CRBSI を確認し、それらのレジストームの特性を明らかにするために、全ゲノムシークエンシングを用いた。中心静脈カテーテルへの細菌定着(すなわちバイオフィルム構築による)を、走査電子顕微鏡によってモニタリングした。
結果
ブドウ球菌(staphylococci)は最も有病率が高い原因菌であり(36/58、62.0%)、黄色ブドウ球菌(Staphylococcus aureus)およびコアグラーゼ陰性表皮ブドウ球菌(Staphylococcus epidermidis)は CRBSI の24.1%、36.2%を占めた。72 のブドウ球菌分離株のうち 59 がメチシリン耐性であった。中心静脈カテーテル/血液培養の分離株のペアの比較ゲノム解析によって、36 ペア中 35 ペアのゲノム一致、ならびに抗菌薬感受性の表現型と抗菌薬耐性遺伝的決定因子の存在の良好な相関性が明らかになった。バイオフィルムは中心静脈カテーテルの 48.6%に存在した。それにもかかわらず、バイオフィルム構築と CRBSI の間に統計学的に有意な関連性は確立されず、ica(intercellular adhesion)オペロンと agr(accessory gene regulator)群の存在/不在はバイオフィルムの表現型と相関しておらず、バイオフィルムがこの医療関連感染症に及ぼす役割を解明するさらなる研究の必要性が浮き彫りになった。
結論
全ゲノムシークエンシングは、CRBSI を確認するための有用なツールであることが示された。中心静脈カテーテルの 42.3%超はブドウ球菌が定着していたが、CRBSI とバイオフィルムの間に統計学的に有意な関連性は認められなかった。
監訳者コメント:
本研究では、バイオフィルムの形成と CRBSI の発生頻度には相関がみられなかったという。バイオフィルム形成の量や場所によっても異なるのかもしれない。基礎的な知見と臨床的な知見が融合した研究は興味深い。
