Automated surveillance systems for healthcare-associated infections: results from a European survey and experiences from real-life utilization

背景

医療関連感染症（HAI）を検出するための自動サーベイランス法の大部分が、研究の場で開発され実施されているので、大規模実施の可能性についての情報は少ない。

目的

欧州の施設および病院における自動サーベイランスシステムのデザインと実施に関する重要な側面について報告すること。

方法

2019 年 2 月・3 月にオンライン調査を以下の対象に電子メールで配信した：（i）PRAISE（Providing a Roadmap for Automated Infection Surveillance in Europe）ネットワークメンバー、（ii）既存の自動サーベイランスシステムに関する欧州の査読付き論文の責任著者、（iii）欧州疾病予防管理センター（ECDC）の全国的感染予防・制御の中心部門のメーリングリストである。実際の実施に焦点を置いて詳細に再考するために、本質的な特性に基づき調査から 3 つの自動サーベイランスシステムを選択した。

結果

調査と選択した 3 つの自動サーベイランスシステムの再考により、方法・アルゴリズム・データソース・対象とする HAI に関して顕著な違いが確認された。自動サーベイランスシステムの大部分は半自動サーベイランスの分類アルゴリズムを使用し、対象とする HAI は主に、手術部位感染症、尿路感染症、敗血症、その他の血流感染症であった。自動サーベイランスシステムは病院職員の仕事量を減少させた。実施の主な障壁は、厳しいデータ保護規制および IT 基盤の構築と維持であった。

結論

欧州の自動サーベイランスは、方法とサーベイランスの対象にばらつきがあることが明らかにされた。比較を可能にし、均質化を促すために、自動サーベイランスシステムに関する今後の発表論文では、ソースデータ、方法、実施状況についての詳細な情報を提供すべきである。
