ゲオバチルス・ステアロサーモフィルス(Geobacillus stearothermophilus)芽胞および SARS-CoV-2 ウイルスの播種実験において実証された短波長紫外線(UVC)によるレスピレータ消毒の効果★

背景
世界的な新型コロナウイルス感染症(COVID-19)パンデミックは、病院における患者数の急増を伴い、大量の呼吸器防護具(レスピレータ)が必要となったため、その不足が引き起こされた。使用済みレスピレータの消毒に短波長紫外線(UVC)を適用することで、安全な再使用が可能となり、レスピレータの不足が低減される可能性がある。
目的
UVC による消毒の適用が、レスピレータを安全に繰り返して再使用することが可能かどうかを明らかにすること。
方法
波長254 nmで発光する低圧水銀放電ランプを備えた UVC チャンバーを用いて、殺芽胞および殺ウイルス作用を評価した。芽胞およびウイルスを播種したレスピレータを、様々なエネルギーレベルの UVC に曝露させた。病原生物の不活化、ならびにレスピレータの粒子透過性およびフィット性に対して UVC が及ぼす影響について検討した。
結果
ゲオバチルス・ステアロサーモフィルス(Geobacillus stearothermophilus)芽胞の生存には、照射量 1.1 J/cm2 の UVC により 5 log10 の減少が認められた。レスピレータの中央部に芽胞を播種したシミュレーションでは、照射量 10 J/cm2 の UVC により 5 log10 の減少が達成された。SARS-CoV-2 ウイルスは、19.5 mJ/cm2 の UVC への曝露により 4 log10 の不活化が得られた。レスピレータの全層を透過させて芽胞とウイルスに到達するように UVC を照射した場合、照射量 10 J/cm2 の UVC を用いて 5 log10 超の減少が達成された。6 倍の光量の UVC への曝露により、レスピレータのフィット性における完全性またはエアロゾル濾過能力に有意な影響は及ぼされなかった。
結論
UVC は、侵襲性が低く効果的なレスピレータの消毒法であり、UVC 処理されたレスピレータの再使用が可能になることが示された。
監訳者コメント:
実験対象として「レスピレータ」という言葉が使用されており、本文からも FFP2 または N95 相当のレスピレータであり、サージカルマスクではない。UVC による殺菌・殺ウイルス効果が確認され、かつ共有使用も可能としている。紫外線である以上、紫外線の当たらない影になった部分は殺菌されないことに注意が必要である。職業感染制御研究会において海外のデータをもとに(1)加湿熱、(2)紫外線(UV-C)、(3)過酸化水素蒸気、(4)時間の使用などの再使用の方法が掲載されているので、それを参考にするのが良い(http://jrgoicp.umin.ac.jp/index_ppewg_n95decon.html)。
