理論的領域フレームワークを用いた臨床医の抗菌薬処方行動に関する決定因子の検討

目的
理論的領域フレームワーク(TDF)を用いて、抗菌薬処方行動の潜在的な決定因子を特定および定量化すること。
方法
カタールの Hamad Medical Corporation に勤務する医師(約 4,000 名)および薬剤師(約 400 名)を対象とする横断的調査。TDF に準拠して開発したオンライン質問票には、個人および施設の人口統計学的特性、抗菌薬処方行動の潜在的な決定因子に関するリッカート尺度による意見が含まれた。分析には主成分分析、記述統計学、推測統計学が含まれた。
結果
計 535 件の回答が得られ、339 件(63.4%)は医師からであった。回答者は主に男性(346 名[64.7%])であった。ちょうど過半数(N = 285、53.3%)が経験 5 年以下であった。主成分分析では 3 成分(C)の解(ガイドラインの遵守[C1]、実践への影響[C2]、自己効力感[C3])が示された。各成分のために抽出した尺度は内部的一貫性が高く(クロンバックのα[Cronbach’s alpha] > 0.7)、尺度の開発の統計学的妥当性が示された。回答者は概して「ガイドラインの遵守」および「自己効力感」に対して高いスコアを付けた。正のスコアの最低水準は、「実践への影響」の成分内の項目と関連しており、特に TDF 領域の環境コンテキストとリソース、社会的影響が中心であった。人口統計学的特性における成分スコアを比較した推測統計学によって、より高学歴で経験豊富な医師は、肯定的な回答をする傾向が強いことが示された。
結論
本研究によって、環境コンテキストとリソース、ならびに薬剤師とキャリアが浅い臨床医に重点を置いた社会的影響は、臨床医の抗菌薬処方を改善し、それによって抗菌薬耐性率を低下させるための行動変容介入の有用な標的である可能性があることが明らかになった。このような介入は、適切に関連する行動変容法に重点を置くべきである。
