シンガポールの主要な急性期病院におけるデルタ変異株による初の COVID-19 院内クラスター：調査およびアウトブレイク対応★

目的
2021 年 4 月にシンガポールで起きた初の新型コロナウイルス感染症（COVID-19）院内大規模クラスターは、主要な急性期病院の一部閉鎖を招いた。本稿では、患者、職員、面会者間の感染と関連する因子の調査、エアロゾルによる伝播が関与する可能性の検討、BNT162.b2 ワクチンと mRNA1273 ワクチンの有効性の評価、クラスター封じ込めの成功に関する報告を行う。
方法
COVID-19 患者の濃厚接触者および影響を受けた病棟を特定し、重症急性呼吸器症候群コロナウイルス‐2（SARS-CoV-2）感染症のサーベイランスを実施した。患者、職員、面会者のコホートを構築し、感染と関連する因子について評価した。患者サンプルの系統発生解析を実施した。病棟の排気フィルターにおいて SARS-CoV-2 の存在を検査した。
結果
症例は計 47 例で、そのうち、患者 29 例、職員 9 例、面会者 6 例、家庭内接触者 3 例であった。すべての感染症がデルタ変異株によるものであった。換気試験では乱気流が認められ、排気フィルターから採取したスワブ検体は SARS-CoV-2 陽性であった。ワクチン接種後のブレークスルー感染が患者と職員で認められた。患者コホートでは、ワクチン接種により COVID-19 感染のオッズが 79％低下した（補正オッズ比 0.21、95％信頼区間 0.05 ～ 0.95）。
結論
COVID-19 パンデミック開始時には既に当院が実施していた感染制御策の強化にもかかわらず、今回のクラスターが発生した。症例隔離、広範囲にわたる接触者追跡、隔離措置による迅速な管理下でクラスターが起こり、結果として、院内個人防護具使用の強化、入院患者および職員に対する定期検査導入、一般病棟の換気の改善を図る院内インフラの変更につながった。

シンガポールの病院で、2021 年 4 月から 5 月にかけて発生した COVID-19 アウトブレイクの疫学調査とその後の介入についての論文である。詳細な記述疫学は大変に参考になった。
