Reduction in the acquisition rate of carbapenem-rsistant Acinetobacter baumannii (CRAB) after room privatization in an intensive care unit

背景

アシネトバクター・バウマニー（Acinetobacter baumannii）は、とくに集中治療室（ICU）において医療関連感染症に関与する主要な病原体の1つである。

目的

ICU の個室化がカルバペネム耐性 A. baumannii 獲得に及ぼす影響を評価すること。

方法

2017 年 3 月から 8 月に、内科 ICU を複数病床区画から個室型に改修した。改修前 18 か月間（2015 年 9 月 から 2017 年 2 月）と改修後 18 か月間（2017 年 9 月 から 2019 年 2 月）に ICU に在室した患者間で、カルバペネム耐性 A. baumannii の獲得について比較した。Cox 比例ハザードモデルを用いて、人口統計学的データおよび併存疾患について補正した。

結果

患者 901 例（延べ入院日数 8,276 患者日）のうち 95 例（10.5％）が、ICU 在室中にカルバペネム耐性 A. baumannii を獲得した。カルバペネム耐性 A. baumannii 獲得率は、改修前の期間（100 患者日あたり 1.87）では、改修後の期間（100 患者日あたり 0.39）よりも有意に高かった（P ＜ 0.001）。多変量 Cox 回帰モデルにおいて、カルバペネム耐性 A. baumannii 獲得は、栄養チューブの留置（補正ハザード比［aHR］6.08、95％信頼区間［CI］2.46 ～ 15.06、P ＜ 0.001）、持続的腎代替療法（aHR 1.66、95％CI 1.09 ～ 2.53、P ＝ 0.019）、ICU 改修後の個室への入室（aHR 0.23、95％CI 0.12 ～ 0.41、P ＜ 0.001）と有意に関連した。

結論

多剤耐性病原体および院内感染の伝播予防のために、ICU の個室型への改修は有効な対策である。

監訳者コメント：

韓国は日本より遙かに CRAB 分離頻度が高く対応に追われている。病室を個室にすることで MRSA、VRE などの耐性菌の伝播を抑制することができることはこれまでにも報じられてきた。この論文では、ICU において、抗菌薬の使用よりも個室管理や侵襲的手技が、CRAB 獲得の要因となっていた。ディスカッションでは、個室に改修した後のスタッフの行動についても言及されていた。
