Specialists’ opinion regarding factors related to wet loads after steam sterilization

背景

蒸気滅菌後の湿潤状態のエピソードの頻度は高いが、このような事象と関連する因子はまだ不明である。

目的

蒸気滅菌後の湿潤状態と関連する因子の関連性の強さを評価すること。

方法：

デルファイ法の適用により、蒸気滅菌に関する専門家が、サイクルパラメータ、滅菌装置、蒸気、対象物の量的負荷、環境に分類した湿潤状態と関連する 37 因子（f.01 ～ f.37）のリストにおいて、関連性の強さのスコアを付与した。5 大陸に分散する専門家 67 名が本研究の全段階に参加した。

結果

乾燥段階での真空度など湿潤状態と関連する特定の因子がより明確になったが、環境と関連する因子など他の因子についてはまだ議論の余地がある。平均スコアがもっとも高かった因子は乾燥段階での真空度（f.12）で、スコアは 4.28 であった。平均スコアが 2.66 ともっとも低かったのは、準備段階（f.05）での真空や蒸気の状況が設定値に達するまでの時間の遅延であった。

結論

湿潤状態の発生と関連する因子の大部分で専門家の見解は異なる。得られた結果は、さらなる研究と標準的要件の確立を可能にするであろう。

「医療現場における滅菌保証のガイドライン 2015」には「乾き度の低い蒸気が滅菌チャンバー内へ導入されると滅菌物を濡らし、蒸気の浸透を妨げ、滅菌不良の可能性が高まる。また、滅菌物の乾燥不良の原因ともなる」と記載されているように、本論文でも高圧蒸気滅菌時の湿潤状態に関する問題がクローズアップされている。しかしながら、その原因が様々であり、まずは専門家の意見を聴取し課題を明らかにしたのが本論文であるが、今後本格的な検証を進めてゆく必要がある。
