混合流換気システムを備えた手術室における被服システムおよび人の活動は、整形外科手術中の超清浄空気要件(≦ 10 CFU/m3)の達成の一助となるか?★

目的

手術室における空中浮遊細菌による汚染度は室内の空気の質の重要な指標であり、この汚染度によって清浄な手術環境を確実なものにできる。本研究の主要目的は、被服と人の活動に応じた混合流換気システムによる超清浄手術室の要件(≦ 10 コロニー形成単位[CFU]/m3)の達成の可能性を明確にすることである。

 

方法

ノルウェー、トロンハイムにある St. Olavs 病院、Emergency, Heart and Lung Centre の混合流換気システムを備えた実際の手術室において、模擬手術中に実験的計測を実施した。5 種類の模擬手術において手術部位近傍の細菌濃度を測定した。5 種類の模擬手術はすべて、手術室での同様の状態を再現できる実際の手術手技に従った。

 

結果

実験結果により、5 種類の模擬手術のうち 3 種類の平均 CFU/m3 は 8.5 であることが確認され、超清浄要件を満たしていた。一方で、他の 2 種類の模擬手術は超清浄要件に達しなかった。手術室で手術スタッフが着用した被服の種類と合わせて手術室での活動は、手術中の CFU 高値のもっとも重要な理由であると考えられる。

 

結論

手術室で適切な被服を着用し、スタッフの活動が少ない手術プロセスにおいて、超清浄空気要件(≦ 10 CFU/m3)を達成することが可能である。本研究は、手術室の外科的微小環境の CFU レベルに及ぼす被服と人の活動の影響を明らかにするとともに、手術チーム用の新たな製品の開発に寄与する。

監訳者コメント

整形外科手術において股関節や膝関節の人工関節手術置換手術は出来る限り術野の無菌状態を保ちながら実施することが必要であり、術後感染症の発生に影響する。しかしながら、完全な無菌状態を作る事は不可能であり現時点では手術手技、手術室への人の出入り、手術時の服装、空調設備環境等を調整し、出来る限り浮遊細菌数を減らすことが必要となる。これまで「層流換気システム」による手術室空調によりこれらの術後の SSI が増えたとの報告もあり、WHO は層流システム推奨していない。ちなみに、ウルトラクリーンの条件は 10 CFU/m3 以下で、「混合流」換気システムでの手術室の空気清浄度の基準は 100 CFU/m3 以下であるが、一方で混合流であっても 10 CFU/m3以下が可能との報告もある。SSI の発生は術者からの細菌の飛散と術野への入り込みが大きく影響することが推測され、本論文では、混合流換気システムを使用しつつ、人の出入りと服装での清浄度を検証をしている。
