Rhinopharynx irrigations and mouthwash with dissolved mupirocin in treatment of MRSA throat colonization – proof-of-concept study

背景

メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の伝播、およびその感染を予防するために、保菌者に対する除菌治療が推奨されている。咽頭保菌は、除菌失敗のよく知られたリスク因子である。黄色ブドウ球菌（Staphylococcus aureus）咽頭保菌は鼻咽頭保菌と関連付けられているが、デンマークで現在推奨されている MRSA 除菌戦略では、鼻咽頭保菌は直接の標的とされていない。したがって、鼻咽頭保菌は、長期間にわたる MRSA 咽頭保菌の重要なリスク因子となり得る。

目的

ムピロシン溶液による鼻咽頭洗浄および口腔洗浄が、治療抵抗性 MRSA 咽頭保菌に対する補完的戦略として実施可能および効果的であるかどうかを明らかにすること。

方法

患者を対象としたオープン非盲検試験を、治療抵抗性 MRSA 咽頭保菌者 20 例を組み入れて実施した。本研究では、患者に対して、デンマークの MRSA 除菌戦略に従った標準治療以外に補完治療を行った。補完治療の内容は、14 日間にわたる 1 日 2 回のムピロシン軟膏（軟膏 22 g/L の 2％等張滅菌生理食塩水溶液）の 37°C溶液による鼻咽頭洗浄および口腔洗浄とした。

結果

患者 18 例（90％）が治療プロトコールを遵守し、重大な有害事象を認めた患者はいなかった。プロトコールに従って本研究を終了した患者 18 例中、15 例（83％）および 7 例（39％）が、それぞれ治療終了後 1 か月後および 6 か月後に MRSA 検査サンプルで陰性結果が認められた。

結論

本研究は、非全身的咽頭 MRSA 除菌戦略において、鼻咽頭および口腔咽頭も標的とすることの実施可能性および臨床的有効性を示している。

監訳者コメント：

MRSA の有病率の低いデンマークからの報告。MRSA の有病率の高い日本では手術部位感染症（SSI）対策として、MRSA 保菌者に対して鼻腔内ムピロシン軟膏の塗布やクロルヘキシジンによるシャワー／入浴が行われている。短期的な除菌率は鼻腔内ムピロシン軟膏でも高いので、SSI 対策として鼻咽頭、口腔咽頭を標的とした除菌の必要性は不明だが、MRSA 除菌戦略の選択肢として今後の検討課題となるであろう。
