Increase in the frequency of catheter-related bloodstream infections during the COVID-19 pandemic: a plea for control

背景

新型コロナウイルス感染症（COVID-19）パンデミック中に、人工呼吸器関連肺炎および菌血症を対象とした院内感染症の発生率については述べられてきた。しかし、COVID-19 がカテーテル関連血流感染症（CR-BSI）の発生率に及ぼす影響に関する情報は非常に限られている。

目的

大規模病院 1 施設において、COVID-19 パンデミックが CR-BSI の発生に及ぼす影響を評価すること。

方法

本研究は、2019 年（パンデミック前）と 2020 年（パンデミック中）の 3 月から 5 月の間の CR-BSI の発生率、病因および転帰を比較した後向き研究であった。

結果

CR-BSI に 1 回以上罹患した患者数は、2019 年は 23 例、2020 年は 58 例であった（1.89 対 5.53/1,000 入院あたり、P ＜ 0.001）。カテーテル挿入から CR-BSI の探知までの期間の中央値は、2019 年の症例では 27.5 日（範囲 11.75 ～ 126.00 日）、2020 年の症例では16.0 日（範囲 11.00 ～ 23.50 日）であった（P ＝ 0.032）。

結論

COVID-19 パンデミック中に CR-BSI の急増が認められた。標準的な予防策と新しい予防策の強化が必要である。

監訳者コメント：

この 1 施設での研究では、COVID-19 パンデミック期間中の CR-BSI の発生はそれ以前に比べ、3 倍になっていた。また、その多くが ICU での治療、ステロイド投与、腹臥位であった。著者らは、カテーテル挿入とケアが増加の原因になったと考えている。貴重な報告である。
