静脈‐静脈体外式膜型人工肺（VV-ECMO）の長期使用例におけるカンジダ血症の発生率★

本単一施設後向き研究では、2014 年 1月から 2018 年 12 月に呼吸補助のため、静脈‐静脈体外式膜型人工肺（VV-ECMO）を受けていた患者 145 例におけるカンジダ血流感染症の発生率の変動について報告している。カンジダ血流感染症の発生率とオッズ比（OR）を算出し、ECMO 使用週数に従って層別化した。ECMO 使用期間中に週当たり発生率は上昇し、OR に上昇の傾向がみられ（P ＝ 0.005）、カニューレ抜去後に継続的なリスクのある期間が認められた。カンジダ血流感染症を発症した患者 13 例中 5 例（38％）は、臨床的疑いによる培養陽性結果が出る前に経験的ミカファンギン療法を受けていた。ECMODiagnostic Stewardship（診断支援）の実践を改善すること、ならびに不必要な抗真菌薬予防投与または経験的管理をしないよう求めることを目的とした前向き研究を実施する必要がある。
監訳者コメント：
イギリスの単一施設での後向き研究。VV-ECMO はカンジダ血流感染症のリスクの増加と関連しているが、包括的な抗真菌薬予防でなく、Diagnostic Stewardship（診断支援）によるアプローチを今後評価していく必要がある。
