イングランドのNHS 急性期トラストにおける院内発症型クレブシエラ（Klebsiella）属菌および緑膿菌（Pseudomonas aeruginosa）菌血症の発生率の上昇：2020 年 8 月から 2021 年 2 月の全国サーベイランスデータのレビュー

2020 年 8 月から 2021 年 2 月の間に、イングランドの院内発症型クレブシエラ（Klebsiella）属菌および緑膿菌（Pseudomonas aeruginosa）菌血症の発生率は、2017 年 4 月の義務的サーベイランス開始以来の最高レベルにまで上昇が認められた。症例は、主要なグラム陰性血流感染症に関するイングランドの義務的サーベイランスのデータベースから抽出した。院内発症型菌血症症例の発生率は、Klebsiella属菌では 100,000 病床日あたり 8.9（N ＝ 255）から 14.9（N ＝ 394）へ上昇し（発生率比［IRR］1.7、P ＜ 0.001）、緑膿菌では 100,000 病床日あたり 4.9（N ＝ 139）から 6.2（N ＝ 164）へ上昇した（IRR 1.3、P ＜ 0.001）（2020 年 8 月から 2021 年 2 月）。これらの発生率は、過去 3 年の同じ期間中に認められた発生率の平均値よりも高かった。これらの傾向は、重症急性呼吸器症候群コロナウイルス-2 にも陽性の院内発症型菌血症症例の割合の増加と一致していた。
監訳者コメント：
COVID-19 の流行にあわせた医療関連感染症の増加は数多く報告されている。この原因はよく分かっていないが、監訳者は医療従事者のフル PPE 着用による①清潔無菌操作の破綻と、②微生物の水平伝播の 2 つが主因ではないかと考えている。日本でも特にコロナ ICU における医療関連感染症の増加が懸念される。
