Development and validation of a competency evaluation model for hospital infection prevention and control practitioners in the post-pandemic era: a mixed methods study

背景

新型コロナウイルス感染症（COVID-19）パンデミック時の院内感染の管理は一層厳しくなった。公衆衛生危機に対応する医療従事者のコンピテンシーモデルを開発することが急務である。

目的

病院の感染予防・制御策に従事する担当者のコンピテンシーを明らかにすること。

方法

文献レビュー、核となる情報提供者へのインタビュー、デルファイ法、質問票調査に基づき、理論的フレームワークを開発した。これらの項目を、回答率、最大スコア、最小スコア、平均スコアにより評価した。コンピテンシーモデルの構造を明確にするために、探索的および確証的因子分析を用いた。

結果

質問票に対する有効な回答率は 88.29％、Cronbach のα係数は 0.964 であった。因子分析により、Kaiser‐Meyer‐Olkin スコアは 0.945 であることが示された。Bartlett 検定では、χ² 値は 10523.439（df ＝ 435、P ＜ 0.001）であった。探索的因子分析により、5 因子モデルが採択され、4 項目が除外され、5 次元 26 項目の指標が得られた。新たな構造の確証的因子分析により、高い適合度（比較適合度指標 ＝ 0.921、Tucker‐Lewis 指標 ＝ 0.911、標準化残差 2 乗平均平方根 ＝ 0.053、近似誤差 2 乗平均平方根 ＝ 0.044）が示された。

結論

提示された指標は病院の感染予防・制御に従事する担当者のコンピテンシーの評価において有用なツールであり、病院の感染予防・制御の改善を促すであろう。
