Three-year hand hygiene monitoring and impact of real-time reminders on compliance

背景

手指衛生は依然として、感染制御における主要な戦略であるとともに、現在も課題となっている。手指衛生自動モニタリングの持続可能性における中心的な問題は、医療従事者の離職率と参加の拒否である。

目的

手指衛生遵守率およびリアルタイムのリマインダーが及ぼす影響を 3 年間にわたり評価すること。

方法

医療従事者の遵守状況は、部屋の出入りの際の擦式アルコール製剤の使用について観察した。時間自動補正を行う線形マルチレベル混合モデルを用いて、部屋における1日当たりの遵守状況の反復測定、ならびに 8 四半期（24 か月）にわたりリマインダーが及ぼす影響を解析した。

結果

全体で、医療従事者 111 名を観察し、データベースにおいて手指衛生活動 525,576 件を特定した。部屋の出入りの両方において 2 年間にわたり遵守率に改善が認められ、リマインダーを作動させた部屋では、リマインダーを作動させなかった部屋と比べて、成績が良好であった。

結論

本研究では、リアルタイムのリマインダーの使用が有益であることが示された。リマインダーを作動させた部屋が 20％でも、手指衛生の全遵守率が改善した。リアルタイムのリマインダーをランダムに作動させることが、ユーザー疲労の軽減および医療従事者の自覚強化における解決策となる可能性がある。

監訳者コメント：

フランスでの 2 年間にわたる手指衛生監視システムによる遵守率の改善の報告である。病室の出入りの際に手指衛生が行われなかった場合、リアルタイムでリマインダーとしてアラームが鳴ることで手指衛生の動機付けになり、遵守率の向上につながる。一方で、アラームが日常化するとスタッフのアラーム疲れが問題になる。アラームが鳴る部屋を減らすことにより、アラーム疲れを軽減するとともにアラームが鳴らない部屋でも手指衛生遵守率の維持につながっている。
