FUrTIHF: French urinary tract infections in healthcare facilities — five-year historic cohort (2014 — 2018)

背景

尿路感染症（UTI）は高頻度にみられ、多様である。重症でない場合でも、UTI は通常、入院につながるが、病院におけるUTI の疾患負担は明らかでない。本研究は、フランスの入院患者における UTI の全国発生率を推定することを目的とした。

方法

フランスにおいて UTI を有する成人入院患者の 5 年間にわたる歴史的コホートを、集学的チームが構築した ICD-10 コードアルゴリズムを用いて、複数の医療・行政データベースから抽出した。成績を評価する変数は、症例 1,122 例の精査により、医療報告をゴールドスタンダードとして用いて盲検下で推定し、全体的な陽性予測値は 70.4％（95％信頼区間 66.6 ～ 74.1）となった。次いで、UTI の全国発生率を推定した。

結果

研究期間中に、合計で UTI を有する入院患者は 2,083,973 例であった。補正発生率は入院患者 100,000 人当たり約 900 例で、研究期間中に一定で、女性のほうが高く、年齢とともに上昇した。1.2％はデバイス関連 UTI であった。原因不明の急性膀胱炎が症例の約 3 分の 2（63.5％）を占め、次いで腎盂腎炎（23.6％）および前立腺炎（12.4％）が多かった。4 分の 3 以上（76.8％）は、少なくとも 1 つの併存症を有していた。

結論

この全国コホート研究は、フランスにおける UTI 関連の入院の発生率を推定した、これまでで初の研究である。UTI は、医療に大きな負担をもたらしている。さらなる分析によって、UTI の各タイプと、その管理および転帰に関して、治療目標に関するより多くの情報に基づく議論のためのデータが提供されよう。
