地域社会における新型コロナウイルス感染症（COVID-19）の症例数の増加によって、医療現場に重大な流行の圧をもたらしている。医療従事者が COVID-19 に感染した場合、接触者追跡調査および疫学調査は感染源を特定するのに十分でない可能性がある。本稿では、以前に COVID-19 アウトブレイクが生じていない病院で患者から医療従事者への伝播が発生したかどうかを明らかにするための、感染した医療従事者 2 名と患者 9 例を対象とした系統学的調査について報告する。これは COVID-19 の有病率が低い地域における院内伝播の疑いを調査するためにゲノム系統学を適用した最初の研究である。我々の結果は患者から医療従事者への直接伝播の発生を裏付けるものではない。
監訳者コメント：
「最小限の仮定で現象を説明せよ」といういわゆる「オッカムの剃刀」の原則からは、COVID-19 アウトブレイクの疫学調査において、関連のない事象を結びつけて説明しがちである。しかし、市中感染のリスクが高い流行期には複数の株が院内で同時に流行する可能性もあるため、ゲノム系統学により異なる株であることがわかれば、直接伝播を否定することができる。
