待機的手術患者における SARS-CoV-2 感染症予防モデルの有効性‐前向き研究：普遍的スクリーニングは意味があるか？★

本観察研究は患者の安全性を維持するために新型コロナウイルス感染症（COVID-19）の術前スクリーニングを受けた患者を対象とした。8,740 件の外科手術中 2,292 件で新型コロナウイルスに対する逆転写ポリメラーゼ連鎖反応（PCR）を実施したところ、PCR 陽性の結果の発生率は 0.0022％であった。医療関連感染症は検出されなかった。前年の同時期と比較して全死亡率または入院期間に差は認められなかった。隔離策、術前の臨床・疫学的評価および COVID-19 関連の予後不良の影響を受けやすい選択された大手術に基づいて PCR 検査対象患者を同定する選択的スクリーニング戦略は、患者と医療従事者にとって効果的で安全である。
監訳者コメント：
COVID-19 の流行に伴い手術患者のスクリーニング検査の有効性を示したスペインからの報告。RT-PCR の真の陽性が 5 例に対し、偽陽性 10 例（汚染、人為的ミス、以前の感染による RNA の断片の検出）があり、治療延期などの偽陽性が患者に与える影響が気になるものの、手術患者のスクリーニング検査は院内感染予防のためには重要な戦略と言えるだろう。
