Automated surveillance in French ICUs: is it feasible? Results from a survey in French ICUs participating in a surveillance network

フランスの集中治療室（ICU）の医療関連感染症、抗菌薬使用（AMU）、抗菌薬耐性（AMR）のサーベイランスを目的として構築されたネットワーク（REA-REZO）内の ICU を対象に、コンピュータ化のレベルを評価するために調査を実施した。ICU 98 室が回答し、患者記録に関するコンピュータ化は57％、抗菌薬処方に関しては 59％、AMR 疫学調査に関しては72％であった。当該 ICU に提供された AMU と AMR のフィードバックはそれぞれ、ICU の 77％、65％で、国内サーベイランスに提供された AMU と AMR のフィードバックはそれぞれ、ICU の 79％、65％であった。本研究により、ICU におけるコンピュータ化のレベルのさらなる改善が必要であることが示唆される。

監訳者コメント：

ICU でのサーベイランスを自動化するには、生体モニター、人工呼吸器や輸液状況の自動取込などを含めた患者記録のコンピュータ化が前提である。大学病院においては他の病院に比較して IT 化は進んでおり、ICU 専用の情報システムが採用されている。抗菌薬使用や耐性菌は、処方データあるいは微生物検査データから得られた情報によりサーベイランスは可能であるが、それらのデータが有効に活用されているかどうかの評価やアウトカムについては十分ではなく解決すべき課題が残されている。
