Bed-days and costs associated with the inpatient burden of healthcare-associated infection in the UK

背景

医療関連感染症（HAI）は罹病率および死亡率の増加と関連し、結果として追加費用が生じる。

目的

あらゆる種類の HAI が入院費用に及ぼす影響を、各種アプローチを用いて検討すること。

方法

Evaluation of Cost of Nosocomial Infection（ECONI）研究の一環として、収集データにより HAI の発生率と種類、入院期間延長について推定した。入院患者延べ在院日数の単位原価を推定するために、スコットランドの国民保健サービス（NHS）の参照費用を用いた。感染予防・制御策および治療と関連する変動（現金）費を、HAIの種類別ならびに全体で算出した。HAI による入院費用を、入院患者延べ在院日数の損失・入院患者延べ在院日数による費用・現金原価の観点から示す。

結果

スコットランドにおいて 2018 年から 2019 年の HAI による入院患者延べ在院日数の損失は 58,010 日（95％信頼区間 41,730 ～74,840）、費用は 4,640 万ポンド（1,900 万 ～ 1億 2,900 万）と推定された。英国の年間総費用は、推定で 7 億 7,400 万ポンド（3 億 2,800万 ～ 21 億 9,200 万）であった。HAI の種類別にみて費用がもっとも高額であったのは血流感染症と肺炎であった。HAI 総費用のうち現金原価の割合は少なく 2.4％の割合であった。

結論

感染予防・制御プログラムの費用対効果を評価する上で、HAI 管理における費用負担の確実な推定が重要である。今回の独自の研究は、確固とした経済データを提示するもので、HAI は依然として英国 NHS への負担となっており、入院患者延べ在院日数は入院費用の大部分を占めていることが確認された。これらの結果は、競合する感染予防・制御プログラムの経済評価および意思決定分析のモデル化のための情報の提供に、地域および国レベルで使用できるであろう。
