個別化感染予防・制御:医療関連感染症リスクのある患者の特定★

背景
医療関連感染症(HAI)研究で、入院時点の HAI リスクに主眼を置いたものは少ない。このリスクを理解することで、入院時点からペイシェントジャーニーの中心に感染予防を置くケアの計画および管理が可能になる。
目的
HAI 発症リスクを高める入院時の患者の内因性特性および入院前 2 年間の外因性事象を明らかにすること。
方法
Evaluation of Cost of Nosocomial Infection(ECONI)研究の一環として、2018 年から 2019 年の 1 年間にわたり、国民保健サービス(NHS)スコットランドの病院 2 施設の成人における発生率調査を実施した。主要評価項目は、広く認められた症例定義に基づくあらゆる HAI の発症とした。ルーチンの院内エピソードデータおよび地域の処方調剤データとの連携によりコホートを抽出した。
結果
もっとも重要とみられた入院時の HAI 発症リスク因子は、教育病院で治療を受けていること、加齢ならびに癌、心血管疾患、慢性腎不全、糖尿病との併存および緊急入院であった。HAI 発症の相対リスクは、集中治療室、高度治療室、および外科専門領域への入室、さらに、入院前 29 日以内の手術、入院前 2 年間の 30 日超の総入院日数に伴い増加した。
結論
入院時点から HAI リスク患者を対象にすることで、感染予防の可能性が最大限に高まり、このことは、外因性リスク因子を知ったうえで管理された場合、とくに顕著である。本稿は、感染予防・制御の新たなアプローチを提示するもので、特定の種類の HAI の発症リスクがもっとも高く、個別化感染予防・制御介入の潜在的候補者となりうる患者を特定した。

入院時点の HAI リスク因子について分析・評価を行っている。基礎疾患として加齢ならびに癌、心血管疾患、慢性腎不全、糖尿病を、入室部署として集中治療室、高度治療室、および外科専門領域、手術歴ありの場合、入院前 2 年間の 30 日超の総入院日数がAI リスク因子としてあがっている。
