Factors associated with independent nurse prescribers’ antibiotic prescribing practice: a mixed-methods study using the Reasoned Action Approach

背景

処方看護師の人数は増加しているが、いまだに処方看護師の抗菌薬処方行動に関するエビデンスはほとんど存在しない。

目的

独立した処方看護師が上気道感染症を初めて発症した患者を抗菌薬を処方せずに管理する意志を評価すること、ならびにこの行動の決定因子を検討すること。

方法

本研究は合理的行動アプローチを用いた混合研究法であった。独立した処方看護師との 27 件の電話インタビューのデータの内容分析は質問票の開発に反映され、質問票は妥当性と信頼性の検査が行われ、スコットランドの独立した処方看護師の全国調査に使用された。抗菌薬を処方せずに患者の管理を行う意志を判定するため、またこの意志に対する有意な影響を明らかにするため、記述分析および推論統計解析を実施した。

結果

参加者 184 名によると、独立した処方看護師は上気道感染症を初めて発症した患者を抗菌薬を処方せずに管理する意志があることが分かった。重要な決定因子は認知された規範、認知された行動管理、道徳的規範であった。有意に信用されたことは、他の非医師の処方者（P ＝ 0.007）および処方看護師（P ＝ 0.045）による前向きな社会的影響、処方者の経験および自信を手助けするもの（P ≦ 0.001）、患者／介護者によるプレッシャーの障壁（P ＝ 0.010）であった。

結論

本結果によって独立した処方看護師は適切に処方する意志があることが再確認できた。この行動を管理する今後の介入において、看護師特有の意志に対する障壁および意志を手助けするものの特定を認識して標的とすべきである。
