背景

ドイツ北東部の病院群における超多剤耐性肺炎桿菌(Klebsiella pneumoniae)株 ST307 の 1 件のアウトブレイクによって、この菌株の疫学的成功は殺生物剤に対する耐性増加に基づく可能性があるという仮説が生まれた。

 

方法

本アウトブレイク株の耐性を、疫学的に無関係の肺炎桿菌の臨床分離株、ならびに緑膿菌(Pseudomonas aeruginosa)(ATCC 15442)および大腸菌(Escherichia coli)K12(NCTC 10538)の参考株と比較した。試験は欧州規格(EN)1040 に基づいて小型化アッセイで実施した。広く使用される殺生物剤の塩化ベンザルコニウム(BAC)および塩化ジデシルジメチルアンモニウム(DDAC)、それらの市販製剤である Descosept spezial(DS)、ならびに消毒薬のクロルヘキシジングルコン酸塩(CHG)を試験物質として選択した。これらの殺生物剤は本アウトブレイクに関わる病院で定期的に使用されている。

 

結果

すべての殺生物剤は、定量的懸濁試験において一般的に使用される濃度と希釈で 5 分以内に全試験株に対して殺菌効果を示した。BAC と DDAC の単独および併用での有効性、ならびに CHG による消毒の有効性は試験条件下で損なわれることはなかった。

 

結論

本アウトブレイク株は殺生物剤の消毒に対して有意な耐性増加を示さなかった。したがって本株の疫学的成功は、訪問者の不十分な手指衛生のような他の原因によるものに違いない。

監訳者コメント

微生物の消毒薬耐性、特に低水準消毒薬に対する耐性は時折話題にのぼるものの、実臨床上明確に問題になることはまれであるし、証明することも困難である。現状は一般的な消毒薬のスペクトラムに基づいて対応するしかないだろう。
