アンジオテンシン変換酵素 2(ACE2)は重症急性呼吸器症候群コロナウイルス 2(SARS-CoV-2)の細胞受容体であることから、ACE2 発現細胞が標的細胞となり、感染しやすくなる。ACE2 受容体は口腔内に発現する頻度が高いので、口腔は SARS-CoV-2 感染の潜在的経路としてリスクが高い可能性がある。さらに、COVID-19 の症状が出現する前でもウイルスは唾液中に検出されることがあり、結果として無症状/発症前の患者によるウイルス伝播のリスクが高くなる。口腔内ウイルス量を減らすことにより、唾液飛沫またはエアロゾルを介した伝播リスクが低減する可能性があり、これはパンデミック制御に寄与するであろう。本稿の目的は、コロナウイルスの不活化または根絶を図るために口腔内消毒薬の in-vitro および in-vivo での影響を試験した使用可能なエビデンスを評価することである。システマティックレビューの実施にあたり使用した基準は、PRISMA 声明に記載されたものであった。Medline(PubMed 経由)、Web of Sciences で電子検索を実施し、以下の MeSH 用語を用いた。‘mouthwash’ OR ‘oral rinse’ OR ‘mouth rinse’ OR ‘povidone iodine’ OR ‘hydrogen peroxide’ OR ‘cetylpyridinium chloride’ AND ‘COVID-19’ OR ‘SARS-CoV-2’ OR ‘coronavirus’ OR ‘SARS’ OR ‘MERS’。初回の検索方法では、2 つの電子データベースで 619 報の論文を特定した。そのうち 17 報にはコロナウイルスに対する口腔内消毒薬の殺ウイルス効果の評価が含まれた。結論として、SARS-CoV-2 ならびに他のコロナウイルスのウイルス量を低減しうる消毒薬の使用を支持するのに十分な in-vitro エビデンスがある。とはいえ、大部分の口腔内消毒薬の in-vivo エビデンスは限られている。臨床的有効性を確認するために、対照群を設定した無作為化臨床試験が必要である。

監訳者コメント:

国内でもポビドンヨードによるうがいの効果が話題になった。この影響か、国内では一部の歯科医療機関で歯科診察・処置前にポビドンヨードによるうがいを依頼するところがある(https://www.jda.or.jp/corona/Povidone-iodine-member.html)。一時的ならまだしも頻回かつ定期的な消毒薬によるうがいは、常在菌叢の撹乱につながることを注意する必要がある。
