本研究の目的は、3 カ所の集中治療室(ICU)におけるカルバペネマーゼ産生腸内細菌目細菌(CPE)によるシンク排水管の汚染について報告すること、ならびに入院患者への伝播リスクを評価することである。培養による CPE スクリーニングのために、すべてのシンク排水管から検体を毎月採取した。入院患者において直腸検体を用いた CPE 保菌スクリーニングを週 1 回実施した。シンク排水管の検体の 22%から CPE が分離された。一部のシンク排水管では、数か月間、同じ菌株が定着し続けていた。研究期間に、入院患者の CPE 獲得はみられなかった。ICU のシンク排水管の CPE 汚染のスクリーニングを目的とした系統的サンプリングなどの特定の対策は、アウトブレイク時を除き、推奨されない。

監訳者コメント

ICUにおける薬剤耐性菌、特にグラム陰性菌はシンクの排水管に定着し、院内感染のリザーバー(供給源)となり、アウトブレイクを引き起こすことが報告されている。しかしながら、排水管への定着は原因なのか結果なのかは議論のあることころであるが、排水管への対応(排水管の交換、消毒、熱処理など)により終息したとの報告もある。本論文では、CPEが長期検出されているにもかかわらずアウトブレイクを引き起こしていないことから、このような排水管への対応についてはさらに議論の余地があるとし、それよりもアウトブレイク中の適切なアルコール手指消毒の徹底による直接的な感染伝播を減らすことが最も重要であるとしている。
